§ 37-55. Required information.

An application for registration shall be filed with the chief of police or the chief's designee and shall be verified under oath and shall furnish the following information:

(a)

The name and address of the applicant and shall state that the applicant is at least 21 years of age.

(b)

The trade name under which the applicant does or proposes to do business.

(c)

If the applicant is a corporation, the officers, directors and principal stockholders of the corporation.

(d)

The vehicle identification number (VIN), year, make and model for each vehicle that will be used in the taxicab business.

(e)

The color scheme or insignia to be used to designate the vehicle or vehicles of the taxicab business with which the vehicle or vehicles will be associated.

(f)

Such further information required by the chief of police and which may be relevant and pertinent to the matter of the issuance of the registration.
